Summary: The Manager of Clinical Operations is responsible for functional and operational oversight and clinical services for Mental Health and Addictions Services inclusive of clinical supervision and service delivery management, quality management and program development/grant management. Individual accountabilities and work volume will be established through the development of annual Success Objectives, within the framework outlined below.

Essential Duties and Responsibilities:

  • Provides clinical oversight in accordance with MHAS core competencies and guidance for MHAS operations, including, but not limited to, participant/case/project assignments, staff employment transactions, implementation of policies and procedures, staff training, etc.
  • Supervises and prepares performance evaluations for assigned staff.
  • In collaboration with Business Operations managers, approves hiring, discipline, promotion, termination and other relevant employment transactions, within defined parameters.
  • In collaboration with MHAS managers, ensures Organization and MHAS policies/procedures for general operations and service delivery. (E.g. personnel, financial, participant eligibility, Standards of Professional Conduct, Ethics, HIPAA etc.)
  • Develops the service plan(s) and service delivery model(s), in conjunction with higher level Manager(s).
  • In collaboration with MHAS managers, ensures a quality assurance/audit system for MHAS, in conjunction with Mental Health & Addiction Services Quality Improvement Plan, and periodically makes appropriate tests of the system.
  • Develops and maintains professional relationships with appropriate groups/organizations, etc., as approved by higher level Manager(s).
  • Remains current in the field of service provided by MHAS through self-initiated avenues, as approved by higher level Manager(s).
  • Provides training to assigned staff in all facets of the position and ensures documentation is completed in a thorough, accurate and timely fashion.
  • Participates in Management Team meetings and acts as Chairperson as scheduled.
  • Conducts internal and external training, as requested.
  • Initiates requests for supervision and interim evaluation/goal setting.
  • Develops long-range plans, periodic assessment reports, success objectives and other significant strategies and/or systems.
  • Ensures the accuracy, substance and timeliness of MHAS reports, projects, schedules and other relevant work products.
  • In conjunction with the Mental Health and Addiction Services managers, designs new programs, implements and evaluates new programs in regards to clinical services according to professional standards of practice and MHAS core competencies.
  • Ensures compliance of MHAS with the standards of the Organization, subsidiary organization, the Medicaid Rehabilitation Option (MRO), Division of Mental Health (DMH), Division of Alcohol and Substance Abuse (DASA), Medicaid and the Rehabilitation Accreditation Commission (CARF).
  • Adheres to professional standards as outlined by governmental bodies, American Psychological Association (APA), National Association of Social Workers (NASW), Illinois Alcohol and Addictions Certification Board (IAACB), and private funding sources. Assists in the development of Organization plans/policies related to professional standards. Provides periodic evaluative reviews and/or in-house and external staff training to ensure that staff understand and continue to adhere to such standards. Initiates requests for assistance or guidance from Supervisor to address new issues or complex concepts affecting adherence to professional standards.
  • Other duties as assigned.

  • Education and/or Experience: Bachelor’s degree in social work/psychology/related human service field and 8 years of experience or Master's degree in social work/psychology/related human service field and four years of experience. Experience in clinical supervision, direct clinical service, program planning and development, quality improvement, and management of social service delivery systems required.
  • License/Certifications: License eligible or current clinical license preferred (Licensed Clinical Social Worker or Licensed Clinical Psychologist). Some positions, due to funding requirements, require Master’s degree and license eligible or current clinical license.

Senior Medical Case Manager

Summary: This position provides a broad range of specialized medical case management services within a clinic environment according to guidelines established by the Organization and specific programs. This position functions with a high degree of independent accountability, in accord with professional and ethical standards, organizational policies and specific contractual requirements. Populations served will vary by program but may include: children; adults; families; immigrants; refugees; and individuals with mental illness, HIV/AIDS and/or substance abuse issues. Individual accountabilities and work volume will be established through the development of annual Success Objectives, within the framework outlined below.

Essential Duties and Responsibilities:

  • Develops and implements program participant treatment plans using experience and understanding of particular chronic medical conditions within a health care setting (medical conditions may vary, some examples include: HIV, Hep C, STI’s, TB)
  • Oversees and implements the plans with program participants to ensure that realistic goals and deadlines of accomplishments are being met.
  • Provides case management services to program participants through both short-term and long-term case management models and a chronic disease model.
  • Receives case assignments, creates case management service plans, reviews case progress and determines case closure through the supervisor of the assigned program and in accord with funder requirements.
  • Meets regularly with assigned cases and representatives from community as needed.
  • Provides case management follow-up for medical adherence of program participants; including access to medication, medication management and adherence, appointment setting and retention in care, and risk reduction strategies.
  • Meets regularly with cross-disciplinary health team to review cases; escalates medical and/or social concerns with medical providers to ensure appropriate medical care decisions and follow-up occurs
  • Provides intense advocacy, crisis intervention and problem solving in a variety of areas for all program participants.
  • Conducts outreach and intake as required.
  • Provides program participants with problem solving assistance and case management support to increase or restore program participant’s capacity for self-sufficiency.
  • Develops service plans, which outline a specific plan of action with each program participant under the supervision of the supervisor of the assigned program.
  • Makes referrals in accordance with service plans to other Organization units and external providers.
  • Screens for income supports for benefits and entitlements to advocate on the behalf of program participants.
  • Provides financial assistance and financial management support to program participants, in accord with Organizational policies and procedures.
  • Accurately completes in a timely manner all necessary forms, case recordings and statistical reports.
  • Develops, monitors and enhances professional relationships with social service, health and governmental agencies.
  • Participates and contributes in regular supervision meetings, team meetings, in house training sessions and conferences.
  • Participates in program and operational planning.
  • Establishes and maintains a professional and therapeutic relationship within the guidelines of professional standards, program participant profiles, confidentiality laws and the newest clinical research available.
  • Maintains the volume of work designated by project standards and utilizes strengths-based theory and concepts integration in clinical practice. On an ongoing basis assimilates the knowledge of resources available to implement clinical concepts.
  • Adheres to professional standards as outlined by governmental bodies, HRSA, NASW (and/or other appropriate professional associations), private funding sources, Organization plans/policies and Unit guidelines. Participates in periodic evaluative reviews and/or in-house and external staff training to ensure that he/she understands and continues to adhere to such standards. Summary: The Community Health Nurse provides quality primary health care service to special populations. The individual is a registered professional nurse prepared with experience as a community health nurse generalist, who has proven nursing and public health practice to promote and preserve the health of individuals, families and groups to improve the health of the community. Within the nursing process framework, the Community Health Nurse performs comprehensive assessment and validation of the reactions and responses of individuals, families or groups to actual and potential problems and provides intervention, evaluation and/or referral to meet identified client needs. Through collaboration with health professionals and other team members, the Community Health Nurse works to implement client care plans and to manage and coordinate continuity of care. The community health nurse participates in activities focused on health promotion, disease prevention, care of acute and chronic illness and restoration of health. The community health nurse is accountable for the outcome of their practice. Essential Duties and Responsibilities:

· Performs a comprehensive and accurate assessment of: individuals – by completing the health history, lifestyle risk assessment, physical assessments and appropriate growth and development assessments and by identifying social and cultural factors that influence health; families - by assessing family specific risk factors, related problems and completing the family risk assessment when appropriate; groups - by utilizing and/or collecting group data such as vital statistics, demographics, socioeconomic, cultural and environmental characteristics and appropriate community members

· Determines relevant diagnoses based on health needs of: individuals - by analyzing appropriate health assessments; families – by analyzing family health assessments and groups – by analyzing group and special population data

· Develops plans at each level of prevention (primary secondary and tertiary) that specify nursing actions for: individuals – by providing direct care to meet physical and psychosocial needs; families – by prioritizing problems and establishing mutually agreed upon goals specific to the stage of family development; groups – by using program planning and group concepts to meet the health needs of the group

· Guided by the plan, intervenes to promote health, prevent illness and restore health for: individuals – by implementing nursing plans independently and prescription treatments and standing orders under the direction of physicians and mid level providers and by facilitating and monitoring client referrals and linkages to other internal and external social services and health care services as appropriate; families – by implementing interventions with the participation of the family members; groups – by implementing programs based on assessed needs of groups and by developing health education presentations and participating in the training of groups of clients and other community providers

· Evaluates responses to interventions in order to determine progress toward goal achievement and to revise the database, diagnosis and plan for: individuals – by evaluating individual client interventions; families – by evaluating family interventions; groups – by evaluating groups interventions

· Applies theoretical concepts as a basis for decisions in practice from the disciplines of: nursing - including community health theory related to individuals and social, behavioral and the physical sciences; transcultural nursing theory - community and public health related to families; community and public health theory related to groups

· Collaborates with other multidisciplinary professionals and community agencies to provide high quality health care

· Coordinates clinic and health care outreach services in conjunction with other team members to meet clients primary health care needs and ensure continuity of care

· Plans, implements and coordinates new community health programs with appropriate supervision from the appropriate program director to meet the needs of special populations

· Participates in quality assurance activities through protocol development, collaborative and peer review by: assisting in protocol development and identifying indicators of quality care; developing protocols and identifying appropriate criteria for the evaluation of care; evaluating standards of care based on the quality assurance process, developing protocols and procedures that guide the delivery of care

· Assumes responsibility for professional development and contributes to the professional growth of others

· Develops in depth knowledge of daily program operations, policies and procedures

· Acts as a preceptor in the orientation of new health care personnel, interns and volunteers.

· Serves as a consultant to other health care providers and agencies, sharing specialized knowledge when appropriate

· Documents comprehensive, accurate and continual data on client records and program reports

· Advocates for special populations through education to the public and consultation with clients, consumers and health care professionals

· Practices within the legal and ethical guidelines established by the Nurse Practice Act, the Code of Nurses, the Social Policy Statement and the Standards of Practice for the Community Health Nurse.

· Adheres to professional standards as outlined by the governmental bodies, American Nurses Associations (ANA), Nurses Association of College of Obstetrics and Gynecology (NAACOG), the American College of Nurse Midwives (ACNM), private funding sources, Agency plans/policies and unit guidelines. Participates in periodic evaluative reviews and/or in house and external staff training to ensure that he/she understands and continues to adhere to such standards. Initiates requests for assistance or guidance from Supervisor to address new issues or complex concepts affecting adherence to professional standards.

· Other duties can be assigned at times in order for HAH to meet the mission and/or funder requests to service our participants.

Summary: Serving as a key member of the Alliance Leadership Team, the Executive Director leads the premier healthcare organization for the unhoused in the Chicagoland area and builds on this foundation to expand the delivery of healthcare services to the most underserved people in the City. The role provides thought-leadership while leveraging industry knowledge and best practices to promote the mission and vision of the Heartland Alliance Health organization and Heartland Alliance as OneAlliance. This critical role supports culture development and management efforts.

Essential Functions:

Strategic Planning/Prioritization & Plan Execution

  • In collaboration with the HAH BOD, establishes the mission and vision of HAH, prepares long- and short-range strategic plans, monitors the quality of participant programs, monitors risk and compliance, and prepares and monitors annual operating and capital budgets.
  • Develops, plans, provides, manages, monitors, and evaluates a complex multi-unit professional services company that meets participant, community, and organizational needs.
  • Monitors federal, state, and local policy developments and creates and implements advocacy strategies. Develops policy recommendations and executes related communications strategies.
  • Remains current on development in services areas through independent study and attendance at seminars, conferences, conventions, etc.

Operational & Programmatic Execution/Excellence

  • Ensures quality and compliance systems and processes meet organizational goals and requirements set forth by funders, accreditation standards, and regulatory bodies.

  • Adheres to professional standards as outlined by governmental bodies, appropriate professional associations, and private funding sources. Assists in the development of plans/policies related to professional standards. Develops and implements company guidelines for professional standards. Provides periodic evaluative reviews and/or in-house and external staff training to ensure that staff understand and continue to adhere to such standards.

  • Attends meetings of the Heartland Alliance and HAH Boards of Directors to interpret programs and policies as requested.

  • Provides routine evaluative and monitoring reports, corrective action recommendations, and quality improvement programs to the BOD and CPO.

  • Ensures the negotiation and management of managed care contracts and contracts with governmental units and other private agencies. Ensures grant funding proposals and fundraising efforts successfully fulfill organizational funding requirements.

  • Oversees the development of program services budgets, approves expenditures according to Organization policy, and monitors the overall company financial performance and budget.

  • Represents program operations internally and externally, directly or through delegation. Initiates and maintains interagency agreements; establishes frequent relations with high-level administrative and professional personnel in the health service field, public and private. Interprets organizational services to the community through direct involvement with public and private groups. Makes presentations at conferences and educational meetings, and leads professional seminars or meetings.

  • Acts with, and at times for, the BOD and CPO in making policy or operating decisions on programmatic organization-wide concerns.

    Talent Management & Development

  • Implements the organization’s personnel policies to encourage professional growth to meet program operation goals.

  • In conjunction with Human Resources, oversees leadership level talent acquisition, talent management, compensation management and evaluation as well as supports staff in conjunction with Directors and with any necessary approval from the CPO.

  • Supervises assigned staff ensuring clarity on work prioritization and impact of work on HAH’s short and long-term objectives.

  • Other duties as assigned and/or required.

Education and Experience:

  • Master’s degree in public health, healthcare administration, nurse administration, social work, business, or related human services or related discipline required.
  • Minimum (15) fifteen years’ experience focused on strategic planning, program planning and development, health service administration and fiscal management, policy development and implementation and staff supervision, required.
  • Exposure to and knowledge of federal funding and government contracts strongly preferred. Experience working in non-profit health sector preferred and post-economic crisis preferred.
